✔️ Dosage & Administration of Zeptide 7.5

Route: Subcutaneous injection.

• Starting Dose: Begin with 2.5 mg once weekly. This dose is only for initiation and not for glycemic control.
• Titration: After 4 weeks, increase to 5 mg once weekly. If further control is needed, increase in 2.5 mg steps, with at least 4 weeks at each dose. The maximum dose is 15 mg weekly.
• Pediatric Use: Not approved for those under 18 years of age.
• Missed Dose: If a dose is missed, it can be taken within 4 days (96 hours). If more time has passed, skip the missed dose and resume the next scheduled injection. Maintain at least 3 days (72 hours) between doses when changing injection days.

✔️ Preparation & Use of Zeptide 7.5

• The syringe cap contains dry natural rubber (a latex derivative); may cause allergic reactions.
• Do not freeze. Discard any frozen product.
• Keep syringes away from light.
• Visually inspect for particles or discoloration before use—do not use if abnormal.
• Do not use a syringe if dropped.
• Do not dilute or mix with other medications.

Injection Instructions:

• Administer once weekly, at any time of day, with or without food.
• Inject subcutaneously into the abdomen, thigh, or upper arm. Rotate injection sites.
• If used with insulin, administer separately (not adjacent), even if in the same body area.

✔️ Drug Interactions

• With Insulin/Secretagogues: Reduce doses to avoid hypoglycemia.
• With Oral Medications: Zeptide delays gastric emptying, which may affect absorption, especially for medications with narrow therapeutic ranges (e.g., warfarin).
• With Oral Contraceptives: Advise switching to a non-oral or barrier method for 4 weeks after starting or increasing the dose. Non-oral hormonal contraceptives are not affected.

✔️ Contraindications

• Known allergy to tirzepatide or any ingredients in the formulation.
• Patients with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

✔️ Use in Pregnancy & Lactation

Tirzepatide should only be used in pregnancy if the potential benefits justify the risks, given the dangers of poorly controlled diabetes. Breastfeeding decisions should consider the mother's need for treatment, the possible effects on the infant, and the benefits of breastfeeding.

✔️ Precautions & Warnings

1. Inform patients about the potential risk of thyroid tumors, especially MTC.
2. Watch for symptoms of pancreatitis—stop the drug if suspected.
3. Lower doses of insulin or sulfonylureas may be required to reduce hypoglycemia risk.
4. Discontinue immediately if signs of hypersensitivity appear.
5. Monitor kidney function, especially in patients with kidney issues and GI side effects.
6. Diabetic retinopathy patients should be monitored for worsening.
7. If gallstones (cholelithiasis) are suspected, further gallbladder assessments are needed.

• Store in a refrigerator between 2°C and 8°C.

• To be dispensed and used only with a prescription from a registered physician.
